On my first stab attempt on my current lich, somehow I managed to stab him twice in a row on the FIRST HEALTH BAR. Not only that, both requiem mods are wrong. I wasn't paying attention whether I received the bonus murmurs twice or just once. Here is the screenshot of the attempt: I could be wrong but I can't find any thread reporting this one.
