DirectX has different feature levels, so the game may require different ones your GPU does not support while DxDiag still shows that it's DX12 compatible (if it supports some of them but not the ones the game requires). I'd need more specific info on your GPU to look it up, but you can look at the feature levels here: https://en.wikipedia.org/wiki/Feature_levels_in_Direct3D . I'd also recommend you're using the most recent version of your GPU drivers, as more recent drivers can provide support to later feature levels if the hardware supports it.
