Load In Falling Through The Map

Twice in a row while loading into the Tactical Alert map I've loaded in falling through the map only to die.

 

While falling I can't see anything but the bottom of the map, can't even access the menu. This happens on occasions but twice in a row at the same map is ridiculous. Also, when I try to revive it'll just immediately kill me again.

I have had this several times when joining an ongoing game. All I can see is the skybox and a basic/default hud. The mini-map shows I am outside the boarders of the level but unable to move.

 

Sometimes waiting to fully load fixes this but I presume it depends on the host and their connection.
