I have a question

I've been gone for a long time.....the last time I played was when mesa came out.

So how does one get unique and rare gearmods? I'm asking cause I haven't gotten one since being back. (The past 16 hours) and i haven't seen any nightmare modes. 

Is the only way to get them is to farm bosses now?

Nightmare modes now happen in alerts, and the nightmare mods drop from some alerts too.
