Cephalon Fragment on radar, but not in map

I am currently scanning the cephalon fragments on various planets, and on Mercury, there is a cephalon fragment that I can see on the radar, but there is no way to get to it. I searched the entire map, and nothing leads to it. Is this a bug, or is it supposed to happen? Also, how would I fix it?

A lot of tilesets have hidden rooms and paths that you just may not know of. That fragment was more than likely in one such room and you just missed the entrance. Some of them can be quite sneakily hidden.
