When PoE shipped there was a bug where if you entered the plains Lotus would immediately tell you the mission failed - however it was a lie and the mission was actually still able to be done. Basically it was a false-warning of Failure. I haven't been to plains in almost 2 months since hitting Ostrom 5, so I don't know if this is the same issue or not.
