This is now happening to my squad too. Possible culprit is I initiated a mercy kill right as the tileset changed? -

It's not impossible, because we didn't search the ENTIRE level. But 11 meters distance at the clostest, and we searched several corridors nearby, and I know all the ways to enter secret rooms (except some of the new one's I haven't seen yet) in the laboratory. So seems like a bugged inaccessible spawn -
Just played an Omnia Fissure for the first time. Alchemy - CAMBIRE (Deimos). Once activating the first Crucible, the first or second distressed valve was somewhere under the ground under the Crucible. A few more pipes spawned, but it wasn't enough to get the Conversion Progress to 100%. It got stuck on 69%, no more valves spawned and we had to abort the mission after around 12 minutes of searching and waiting. Screenshot of the room:
