Having this issue as well, have also submitted my own post.

I happened to be running Kuva Survival and picked up a Kuva Catalyst. While holding the Kuva Catalyst and Lex Prime, I WAS able to toggle in and out of Incarnon mode for the Lex Prime. However, I was not able to use the Kuva Catalyst on a Life Support Module while in Lex Incarnon mode. As soon as I toggled out of Incarnon mode, the Kuva Catalyst was applied. Switching, then, to my Glaive Prime + Lex Prime combination, in the same mission, I was again unable to enter Incarnon mode, further leading me to believe this is a Glaive + Sidearm-related bug. Edit: I entered the Downed state, and, due to only having the Lex Prime equipped while downed, I was also able to freely enter/exit Incarnon mode.

I am using the Soma Prime primary, and am able to toggle to it's Incarnon mode without issue, so I believe this to be perhaps a unique bug when using a sidearm + glaive combination?...

While using the Lex Prime and the Glaive Prime, I am unable to toggle to the Lex Prime Incarnon mode. The bar will charge, and even make the audio cue when fully charged, but I am unable to toggle to Incarnon mode. This issue persists between missions.
