When scanning enemy 20 time it to much why can we scan them one time to complete the Endex, And i mist out scanning Zealot and  Scarlet Spear

In-game encyclopedia called "Codex". I guess you probably misspelled it with "Index" - a special mission mode where you can earn a bunch of credits effectively.

It is 20, yes, it is grindy game, there is really much stuff to farm for! As for scanning aspect, there is a way to make your life easier: Helios or Helios Prime Sentinel which can equip Investigator Mod and will automatically scan incomplete things for you!

As for Zealoid Prelate fight (and scanning him, of course), you can buy a blueprint for Orokin Derelict Emissary Assassination key from the market, then craft it and you will get access to the boss fight.

And Scarlet Spear, I believe, is recurring event, so it will happen again eventually. My guess is every half year or one year.
