Requiem Ultimatum failed to spawn my Kuva Lich

A bit of background on my situation, I was going to finish my words on my lich as I have figured out my order the previous mission. Switched my mods around and went into Pallas on Ceres. I popped into a mission and hit my beacon, waited and nothing. In fact another lich different than mine spawned. Exited and lost a beacon.

 

I was a client in a full public lobby running a mesa prime with kuva chakkhurr, lex prime and kogake for some MR with zenurik focus. If I remember correctly the tile I hit the beacon on was the large tile with a pool of acid at the bottom with a bunch of open space and conveyor belts.
