I would go into missions needing a scan but would find the somachord and it says I've already scanned it without being rewarded for it. If Helios is in the middle of scanning something and then you go into Op/Drift the scan seems to instantly complete but you don't get credit for it. Staying in my frame the entire time is the only way to get credit for said scan. I can also reproduce it on demand now that I am 98% sure that is what is happening.
