Assassinate used on lich/sisters without initiating the sequence

Since coming back a couple weeks ago, I've noticed that while hunting liches and sisters that when they are downed (on each health bar I've seen this) the action to assassinate them automatically triggers when approaching them. This sometimes leads to attempts on them before I have any parazon mods actually equipped, pushing progress back further on the attempt.
