Mission Items Not Saving?

I just did an Orokin Derelict Vault run and the mods that I got during the level and the mission reward aren't showing up in my mod inventory. I distinctly remember the mission reward being Corrupt Charge and it isn't showing up in my mod inventory. As well, I used my extinguished key to open the vault but my gear menu shows me having not used a key. It's as if I hadn't done the mission at all or something?

 

The odd thing is, there was no dialogue box saying that mission rewards weren't saved or anything of that sort. It just seemed like a regular mission ending.

 

This same thing happened a few months back when I got a Fever Strike and I didn't notice that it was missing until a couple of weeks after the fact. It's kind of a shame.

 

Is this just a fact of the game or should I be filing a ticket to get the items?

Same here: i just get Bleeding Willow stance and after mission it wasn't in my inventory.
