Nightmare Missions Sometimes Start Wthout Nightmare Debuffs

As the title says. I've been playing on Saturn today, the same mission over and over, and, in like 3 - 4 hours of repeating the same mission, I had 3 instances where there was no debuff at the start or for the rest of the mission (that is, if you don't consider increased enemy level a debuff, and I don't). There are no specific ways to trigger this one, I just kept playing the same mission over and over and it sometimes forgot to trigger.
