Problems Exiting Menus

For the last couple days I've been running into situations where I can't exit out of menus.  The mostly happens after mod fusion.  The list of mods will be blank, and "cancel fusion" will be the only visible button, but it doesn't do anything.  Clicking my mouse back button also fails to exit back out of the menu.  I'm often able to use the escape menu, switch to equipment, and from there I can exit.  However, I just payed the sacrifice fee using the Syndicates terminal on my Cephalon, and the "I N I T I A T I O N Your initiation is complete. You have earned the first sigil!" message came up and wouldn't go away.  I used the escape menu, and selected Navigation.  It took me to my planets menu, but the initiation message was still present.  I was able to use my mouse back button out of navigation, but when I returned to the bridge the only thing I as able to do was type in the global chat.  I had to Alt+Tab out of the game and kill it using task manager.

 

I took a copy of my EE.log file, and have a screenshot of my bridge with the message and no menu options, but I don't see any way to attach files to this post.
