Ash's fatal teleport is broken?

Maybe I'm doing something wrong, I don't know. It'll work on one enemy just fine, but then I teleport to the exact same type of enemy a foot away and I wind up just standing like 2 feet behind them. It seems to be completely random when it actually wants to do a finisher. I'll have a good streak going where everything is great, then total failure. Is there something I'm not doing right?

If you end up too far away from the target, try walking up to them; most times it'll initiate the finisher. This is why I hold forward any time I use FT.
