Do I get a buff for every single slash proc on an enemy or only one buff for multiple slash procs?

Only one buff for multiple slash procs. The damage increases from having multiple types of procs on a single enemy, not multiple instances of the same proc.

Add to that: Once the Buff run out (duration), you can re apply for that Buff.
