Mysterious failed slam attacks

I cannot figure out what the exact cause of this bug is, but it only seems to happen on Dedicated Servers.

The bug is that on rare occasions, a slam attack will instead result in the person hard landing, rather than a normal slam attack.

Update - Player seems to get stuck "looping" the unholster animation. Never actually pulls the melee weapon out, and as such, smashes into the ground at slamming speed - but without being able to slam at all.
