Stuck/Not working controls

Is it me or the game's controls is the one that is bugged? 

I have an issue wherein i can't shoot or block.

For example:

I have my Grakatas. Whenever i try to shoot, it won't. But when i press the shoot key and alt tabbing out and back in, it functions normally now but at the cost of other weapons not working properly, 

another example is

I have my melee out, I want to block projectiles.

I switched to my melee and my block button is RMB.

Pressed RMB and blocked projectiles

I can't return to my idle stance even though my toggle is turned off

The only way of fixing the problem is holding RMB simultaneously alt tabbing in and out of the game.

and switching other weapons would cause it to not shoot.

I did my virus/malware scanning and found none.

I did try uninstalling the mouse driver and unplugging it and plugging it again and it failed to cease the problem.
