I have been having this problem as well. Mine has a toxic kuva grattler. A weird interaction also has happened when i summoned him. I had died just a little before summoning him and he stood still and didn't attack or follow. Just used his abilities. However, the normal "kuva lich here to help you" event happened with that kuva lich. Which activated him and made him work properly. Even gave him the normal blue kuva lich icon.
