Same problem, trying to do plague star and needing to do solo most of the time because of this problem.

Sometimes the Isolation Vault door puzzle glitches out and shows all the symbols the same, making it impossible to see the correct ones. I entered Deimos by invite last time, maybe this help out replicating it.

This also happened to me when someone pressed the button, twice, needs a fix asap.

Predasites mesh are disconnected on tails and back of the ears, leaving a hole on them.
