The communication with the game was somehow interrupted and you are seeing placeholders instead of actual factors in the progress. If so, would argue it's not a bug. Not something to be fixed, as it will always react the same way in the same situation. Do you have a heavy weapon equipped? Perhaps the summary screen provides a placeholder for an expected item due to not entirely receiving the info from your side of not having one. I don't know if there is any more nuance to it, I just tested it myself. Nevermind, it's when you go into operator for the first time in a mission.
