Teach me how to properly report bugs please...

First time posting in the bug report forums so hi hi... Title pretty much sums it up. I want to post some issues I have found and am wondering what to bring to the table... Normally I just work around any issues and leave it to someone else to report... But if I'm going to bother at all I'm going to do it right.

PS an official report submission form, from an actual tester would help me understand WTF I need to know to share issues properly.

Full description of the bug - the more pertinent information you can provide the better.  Is it intermittent?  Is it pervasive?  Can you reliably trigger the bug to occur?  Do you have screenshots or video of the bug occurring?  Were you client or host in a squad?  Solo?  Etc.

Really, give as much detail as you would want if you were on the other end of the screen and trying to figure out how to fix a bug that's been reported.
