Can't switch to operator

So, this is super annoying.

I can't switch to operator sometimes. As in, I try to switch to operator and it instantly puts me back into my frame. It screws missions (like tridelon) among other things.
Dying doesnt' fix it, switching to necramech doesn't fix it. WTH is this bug?
I don't know why this happens but it does, and then the mission becomes nigh impossible afterward.

Note:
I do have all quests other than glast gambit done.

I've been having this happen as well, especially on the Zariman (Last two missions in a row, in fact). Makes it impossible to kill Thrax. THOUGH, it does make it so when you enter the Void Angel bubble (if it lets you, sometimes it kicks you out again), you're in your frame and can just blast it with a good gun which isn't bad.
