Safeguard switch effect persisting

The visual effect for Safeguard switch, the glow pulsing from head to toe, will randomly get stuck on for the rest of a mission, making it hard to tell if the invulnerability is still on or not. Once stuck, it will remain until extraction. It will go back to working at the start of the next mission. No specific action seems to trigger this to happen, just that the more you use swap, the more likely this occurs.
