Caliban summons only 2 sentients instead of 3

It doesnt always happen, but most of the time, caliban only spawns 2 sentients instead of the full trio. I checked if its an energy issue and it isnt because it happens even at max energy.

It seems like the terrain might be blocking the other summons or something.

I commonly see one of the summons die on cast, which really sucks for the single summon.
