You're not likely to find it in three days of playing her. But it does happen, and it's gamebreaking. If you're killed/downed while frozen like that and you're revived, you'll be unable to cast abilities. Happened to me around 30 mins on Void Survival once and it was painful, so we had to leave early. Certain enemies also completely ignore the stagger component of SQ. Seekers simply waltz right through it and to one of your teammates. And that's not an online issue, either. I've tested that in solo and it still happens.
