Limbo Theorem Quest Keeps Disapearing After Clearing A Mission Without Obtaining The Blueprint

So i login and start the mission The bug occurs when if i clear or fail the mission without completing the objective (gathering the proofs, or blueprints from the excavation missions).

 

I have to restart game to make the quest re-appear.

 

Edit: This has been happening to me at all stages during the quest. from earth stage -> archwing stage -> phobos stage -> archwing stage -> europa stage.
