Can't continue Cephalon Codex Entry, what do I do?

I've been working on completing all the codex entries for Cephalon Fragments, but I can't continue Derelict's 3rd entry because the Cephalon Fragment keeps returning I already "completed the codex entry" while I have only taken 1 out of 7 or 8 required scans. I would really like to finish this entry. Is there a way around this or to fix this?

If it's relevant, there were also other things that appear out of the ordinary concerning the Cephalon Codex entries. I had never seen any of the Transmissions until I suddenly got to see the 26th, and I currently have the first scan of two Eris Fragment Codex Entries.

Then try finding Fragments in planets you've already completed. I'd start with Eris, as I suspect there's a link there...

but it's derelict only : / 

I'll give it a shot I guess......why don't they just fix the damn thing already? >.>
