It seems to happen when spam the ability is spammed continuously, though I am unsure of the exact trigger. I've replicated it in mission, and here is a clip of an unmodded (only primed reach on) plasma sword wrecking lvl 165 corrupted heavy gunners in the simulacrum.https://streamable.com/yd0cuAnother clip if the first wasn't convincing enough A fix is mentioned in a PC patch, but it seems like it should have shipped with Xbox's patch based on when when the fix happened on PC.
