Lokidus_Prime Posted January 14, 2022 Share Posted January 14, 2022 I've had this happen with Cephalon Suda medallions on Deimos tilesets as well. One run i had 3 inaccessible medallions - 2 of them were in closets and 1 was stuck in a wall in that one bi-level circular room (spot was near a shockwave deathorb on the 2nd floor, forgot to grab a screenshot though).
