Controller rebinds to D-pad not working

I've had my powers mapped to my D-pad for a number of years and it has worked without much issue.  
After being forced to rebind them to be able to play the Kahl mission, they are now broken; specifically the D-pad Down button to which I generally map Ability A and which coincidentally is the one I was forced to rebind.   
Powers can be cast with the usual mode switch button + face buttons, but my customized binding for this one specific button are ignored. This happens in regular as well as spoiler mode form. 

I'm also getting inconsistent behavior with Melee Heavy Attack sometimes being interpreted as activating Transference but have been too tired of this to investigate further. 

I've reset to default and rebound, restarted the game, switched controllers etc.
