Problem with the relics

There's this problem with the relic when I open them in a mission I don't get corrupt or my weapon dose not turn gold and when the mission ends the game dose not register my relic as open as if I never collected all ten fissures I lose a relic and I never get my item or choose any has anyone have this happened to them?

But anyways, sometimes the relics default me back to my original reward even though I chose a different one.

I've had that once or twice, but I always seem to get the relic back? 

(I checked as it was a radiant)
