It always happens to me when reflex guard kick in or when I block Or when doing a reload on primary relex guard kick in. I was doing a mobile defense mission and was the carrier, and my secondary wouldn't fire at seemingly random intervals despite having full ammo. The animation would occur, but nothing was fired, and enemies weren't taking any damage. At the moment, I'm just guessing it was a transient bug and will keep an eye on it and see if it recurs. If it's a one time thing, it's probably just that and this will be the end of it. But if you find it happening often, raise it as an issue.
