Rescue (Submersible) Hostage AI needs improvement

I was running Syndicate mission which is rescue (submersible) in uranus, i brought the hostage around to look for medallions, and there's one dead end room that have water and may spawn the underwater red bomb or a crate, i just picked a medallion there (not in water) and the hostage walks around circling me and somehow drops itself into water and instantly dies, no dead body around.

Submersible Hostage AI should have a precept to avoid waters seriously. It resets my riven challenge due to mission failure :(

The Syndicate NPCs that accompany you can also fall in and drown the same way.

This issue was addressed before a year ago. I believe there was a hotfix that said it fixed this, but from my experience what it seemed to have done is lessen the chances by a lot how easily the hostage falls in when near water. It can still happen but not so easily as before. Its AI path-finding was tweaked but needs some more adjustments.
