What am I supposed to do when I'm dead?

Specifically, when there's no one in my squad and I'm all alone?

It doesn't give me a bleedout, only just a revive option and that's it. I can't forfeit the mission and I'm supposed to revive and abort or just keep dying?

What happened to the forfeit mission button upon death?

If your alone in the squad there is no one there to pick you up. You will ultimately just die. You get 4 revives a game and every time you use 1 it reduces the affinity you will earn by the end of the game. Honestly if your close to completing the mission revive. If the mission seems impossible just leave. When you forfeit it will just send you back to your ship. This way you can revise your builds or just find squad members to run with

I guess they were going for a cleaner look upon death. But yea, you are supposed to revive and abort, so you still get penalized for dying.

Now that you get a full revive refill every time you return to the Orbiter,

that Abort button - which was way too easy to press & confirm accidentally - being removed was the correct choice, IMO.
