Not sure what this mod is asking of me

So I got my mod from the quest and it's telling me to open 4 caches without dying or going down. I ran a sabotage and found 2/3 chaches and didn't die once, and yet the mod was unchanged. Am I missing something?

Make sure it is equipped on a weapon while you do it.

^What immolator said, as with upgrading frames, weapons, dogs, cats, sentinels and focus you actually need to take the gear with you to upgrade it.

oh... so that's how riven mods work... been trying to figure it out, mine is to finish a survival with lvl 30 enemies or higher without killing any of them... when it refused to unlock after the third mission with lvl 50 enemies i was beginning to think i was the victim of a glitch...

Aah that one I did last night but with a partially different condition and that is a pet is needed to be present while you unlock the caches and for your case you cant bleedout or die from first cache opening till the fourth no matter how many missions are involved; just dont lose all your life or die and that will do the trick for you.

And of course you must equip the mod according to its category otherwise all will go in vein.
